Parishioner Update Form

Registered Parishioners:   Please help us in the church office keep up with you!   It helps us tremendously in the front office when you let us know about changes in your household.   i.e.   new address,  new phone,  dropped land-line,  new email,  an out-of-parish move, a change in marital status, a new baby or child or other family member joining the household,  any sacramental info. (unknown to us).   By completing & submitting the form below when changes occur, we can update our records with your most current information.   When adding a new family member to the household, be sure to let us know his/her full name (first, middle, last), the person's gender, date of birth, religious affiliation, and any sacraments received.   We appreciate your assistance in helping us keep our records up to date!   Or feel free to email the information directly to our front office at:   dgibson@icc-msh.org
